The DV Lottery requires the photo to be exactly 600×600 pixels (or 1200×1200) — square. Files at 800×600 or 600×800 are auto-rejected. JPG format, 240 KB maximum.

Duplicate detection — DV photo cannot be reused
Each DV Lottery cycle, the State Department's system compares submitted photos against the entire previous cycle's photo database. Photos that match a previous entry are rejected as 'submitted previously'.
